Once a hearing evaluation is complete, we can determine the extent of the hearing problem and make specific suggestions to improve your situation.

FINANCIAL ARRANGEMENTS AND INSURANCE
Thank you for choosing us as your hearing health care provider. If you have hearing evaluation or hearing aid benefits through your health insurance, please advise us. We are anxious to help you receive your maximum allowable benefits. In order to achieve these goals, we need your assistance and your understanding of our payment policy.
Payment for services is due at the time services are rendered unless other arrangements have been made in advance with our staff. We accept cash, checks, MasterCard, Visa or Discover Card. We also offer interest free financing for 12 months using the Citi Health Card.
We will be happy to help you process your insurance claim form. We will submit the claim to your primary carrier. If the claim is not rolled-over to your secondary insurance carrier, the balance is your responsibility. We file electronically whenever possible.
The following is a list of some of the companies we are providers for: Anthem Blue Cross and Blue Shield of Indiana, Blue Cross and Blue Shield of Michigan, Blue Cross and Blue Shield of New Jersey, CIGNA, Aetna, and Meritain benefits plan for AM General employees.
Please Note:
1. Your insurance is a contract between you, your employer (or former employer if you are a retiree) and the insurance company. We are not a party to that contract.
2. Not all services are a covered benefit in a specific insurance plan. Some insurance companies arbitrarily select certain services they will not cover. It is your responsibility to call your insurance company to see what your benefits are.
We must emphasize that as hearing health care providers, our relationship is with you, not your insurance company. While the filing of insurance claims is a courtesy that we extend to our patients, all charges are your responsibility from the date services are rendered. We realize that temporary financial problems may affect timely payment of your account. If such problems do arise, we encourage you to contact us promptly for assistance in the management of your account.
